📚 Key Responsibilities

Direct Patient Care

  • Home visits and offering office-based care to mothers and babies and offering disease prevention, health education, and wellness promotion.

  • Apply the nursing processes of assessment, planning, and dispensing care to individuals and groups within the parameters of the program.

  • Exercise professional nursing judgment to provide client-centered care.

Community Health Engagement

  • Take an active role in community health planning and represent maternal child health in community forums.

  • Educate the families on preventive health care, physical and mental health, and healthy lifestyle practices.

  • Serve as a broker between family and community health resources in order to access support services.

Interdisciplinary Collaboration

  • Collaborate with physicians, social workers, community agencies, and other healthcare professionals to coordinate care plans.

  • Build and maintain healthy relationships with community organizations to attain optimal health.

  • Share information and news at interdiscipline meetings to enable collective problem-solving.

Program Evaluation and Coordination

  • Support the Babies First and Cacoon Programs through the upkeep of up-to-date patient records, trending, and assessment of outcomes.

  • Contribute to program enhancement by offering data collection and analysis, and comment.

  • Draw up reports and documents as required by the health department.


🎓 Minimum Qualifications

  • License:
    • Must hold a valid Registered Nurse (RN) license to practice in the State of Oregon.
  • Driving Requirements:
    • Valid driver’s license with a clean driving record is mandatory.
  • Language Proficiency:
    • Bilingual in Spanish is required to effectively communicate with diverse populations.
  • Skills and Abilities:
    • Proficient in community health nursing practices, health education, and patient advocacy.
    • Strong ability to build trusting relationships with clients and collaborate with multi-disciplinary teams.
