The Linn County Department of Health Services is actively seeking a passionate and qualified Public Health Nurse to join the Maternal Child Health team. 📚 Key Responsibilities

  1. Direct Patient Care:
    • Perform home visits and office-based care to mothers and infants, focusing on disease prevention, health education, and wellness promotion.
    • Implement nursing processes to assess, plan, and evaluate care for individuals and families within the program’s scope.
    • Utilize professional nursing judgment to deliver client-centered care.
  2. Community Health Engagement:
    • Actively participate in community health planning and represent maternal child health in community forums.
    • Educate families on preventive healthcare, mental and physical well-being, and healthy lifestyle practices.
    • Serve as a liaison between families and community health resources, ensuring access to support services.
  3. Interdisciplinary Collaboration:
    • Work closely with physicians, social workers, community agencies, and other healthcare professionals to coordinate care plans.
    • Develop and maintain effective partnerships with community organizations to optimize health outcomes.
    • Share insights and updates in interdisciplinary meetings to foster collective problem-solving.
  4. Program Coordination and Evaluation:
    • Support the Babies First and Cacoon Programs by maintaining accurate patient records, tracking progress, and evaluating outcomes.
    • Contribute to program improvement through data collection, analysis, and feedback.
    • Prepare reports and documentation as required by the health department.

🎓 Minimum Qualifications

  • License:
    • Must hold a valid Registered Nurse (RN) license to practice in the State of Oregon.
  • Driving Requirements:
    • Valid driver’s license with a clean driving record is mandatory.
  • Language Proficiency:
    • Bilingual in Spanish is required to effectively communicate with diverse populations.
  • Skills and Abilities:
    • Proficient in community health nursing practices, health education, and patient advocacy.
